Notice of Interpretive or Policy Statement

     In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

     Document Title: Provider Notice #12-71.

     Subject: Medicaid program of HCA will implement a drug formulary.

     Effective for dates of service on and after October 1, 2012, the medicaid program of HCA will implement a drug formulary. Drugs designated as nonfurmulary [nonformulary] will be noncovered services for medicaid clients, and claims for nonformulary drugs will be rejected as noncovered.
