Document Title: # Memo 11-03.Subject: Family planning provider billing instructions: Coverage table update.
Effective Date: February 1, 2011.
Document Description: Effective for dates of service on and after February 1, 2011, DSHS will discontinue paying for procedure code T1023 intake assessments for family planning and TAKE CHARGE clients.
