Subject: Kidney Center Services Medicaid Provider Guide.
Effective for dates of service on and after January 1, 2014, the medicaid program of the health care authority (HCA) will publish an updated Kidney Center Services Fee Schedule and a revised Kidney Center Services Medicaid Provider Guide.
