Document Title: # Memo 10-80.Subject: Kidney center services: Coverage table and fee schedule changes.
Effective Date: January 1, 2011.
Document Description: Effective for dates of service on and after January 1, 2011, DSHS will update:
? The kidney center services fee schedule; and ? The coverage table in the kidney center services billing instructions to reflect changes discussed within this memo.
