In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the department of social and health services.

Document Title: # Memo 10-76.Subject: Blood bank services: Fee schedule and coverage table updates.
Effective Date: January 1, 2011.
Document Description: Begin using the year 2011 current procedural terminology (CPT¦) and healthcare common procedural coding system Level II and modifiers; and
Update the blood bank services fee schedule and coverage table to reflect changes discussed within this memo.
