In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

Document Title: Provider Notice #12-93.Subject: Maximum allowable cost update.
Effective for dates of services on and after January 1, 2012, HCA will update the Physician-Related Services/Healthcare Professional Services Medicaid Provider Guide with new code for Brentuximab vedontin inj, J9042 and authorization changed to covered with PA.
