     Subject: Physician-related services/healthcare professional services medicaid provider guide.

     Effective for dates of service on and after January 1, 2013, the medicaid program of HCA will revise the physician-related services/healthcare professional services medicaid provider guide with procedure code additions, removals, and revisions.
