Notice of Interpretive or Policy Statement

     In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

     Document Title: Provider Notice #12-92.

     Subject: Prescription drug medicaid provider guide.

     Effective for dates of service on and after December 19, 2012, the medicaid program of the HCA is publishing a revised prescription drug program medicaid provider guide. Specific changes to the guide include:

?	Billing for compliance packaging.
?	Billing for vaccine administration.
?	Buprenorphine for pregnant women.
?	Mental health polypharmacy.
?	Step therapy for inhaled long-acting beta agonist/corticosteroid combination drugs.
?	Client eligibility for Synagis¦.
?	Requirements for Vivitrol¦ (naltrexone IM).
