     In accordance with RCW 34.05.230(12), following is a list of policy and interpretive statements issued by the health care authority (HCA).

     Document Title: Provider Notice #12-98.

     Subject: Kidney centers medicaid provider guide.

     Effective for dates of service on and after January 1, 2013, the medicaid program of HCA is: Replacing procedure code Q2047 with procedure code J0890; and updating the kidney center services fee schedule with new maximum allowable fees.
