- Each application for a license, or renewal of a license, as a certification authority shall be submitted on a form prescribed by the secretary. The completed form shall contain the following:(1) The name of the applicant;(2) The applicant's uniform business identifier number, if any;(3) The mailing address of the applicant, and a physical address if different;(4) The telephone number of the applicant;(5) The electronic mail address of the applicant;(6) The name and address of the applicant's registered agent for service of process, other than the secretary. Address information shall include a physical address, but may additionally provide a mailing address if different;(7) The names of all operative personnel; and(8) The appointment of the secretary of state as the applicant's agent for service of process.
