Section 388-845-3061. Can a change in your person-centered service plan/individual support plan be effective before you sign it?

- If you verbally request a change in service to occur immediately, DDA can sign the person-centered service plan/individual support plan and approve it prior to receiving your signature.(1) Your person-centered service plan/individual support plan will be mailed to you for signature.(2) You retain the same appeal rights as if you had signed the person-centered service plan/individual support plan.[Statutory Authority: 2014 c 139, 2014 c 166, 2015 3rd sp.s. c 4, RCW 71A.12.030, and 71A.12.120.
