Section 388-828-1640. What are the mandatory panels in your DDD assessment?

After DDD has determined your client group, DDD determines the mandatory panels in your DDD assessment using the following tables. An "X" indicates that the panel is mandatory; an "O" indicates the panel is optional. If it is blank, the panel is not used.

(2) Supports intensity scale assessment

(3) Support assessment for children

(4) Common support assessment panels

(5) Service level assessment panels
