Section 388-112-0240. What are the documentation requirements for continuing education?

  • (1) The adult family home or assisted living facility must maintain written documentation of department approved continuing education in the form of a certificate or transcript:
    (a) The name of the student;
    (b) The title of the training;
    (c) The number of hours of the training;
    (d) The assigned curriculum approval code;
    (e) The instructor's name and signature;
    (f) The name of the home or training entity giving the training; and
    (g) The date(s) of training.
    (2) The student must be given an original certificate or other documentation of continuing education. The adult family home or assisted living facility must keep a copy of the certificate or transcript on file as described in WAC 388-76-10198 (for adult family homes) and as described in WAC 388-78A-2450 (for assisted living facilities).
