- (1) As a condition of obtaining and maintaining a valid operating permit, owners shall provide evidence of having liability insurance.(2) The minimum amount of liability insurance required shall be one hundred thousand dollars combined single limit. The coverage for this insurance shall include:(a) Bodily injury or death of one or more persons in any one incident from the use of the RWCF.(b) Tail coverage shall be required twenty-four months beyond the insured period on a "claims made" form of insurance.(3) A certificate of insurance shall be provided to the department or local health officer at the time of application for operating permit subject to the approval of the risk manager of the state of Washington.(4) The liability insurance company shall provide the department or local health officer a thirty-day prior notice of cancellation, alteration, or nonrenewal. This condition shall be stated in the certificate.(5) If the owner's insurance is cancelled, the operating permit is void and the owner shall cease operation of the RWCF until required insurance is obtained and a valid operating permit is reinstated by the department or local health officer.
