- UST system owners and operators shall maintain records documenting all currently designated Class A, Class B, and Class C operators at an UST facility and the training received by those operators. The records must be maintained and made available in accordance with WAC 173-360-210(3).(1) Designated operators. Records documenting Class A, Class B, and Class C operators at an UST facility must include the following information:(a) The facility's name, address, and compliance tag number; and(b) For each individual designated at the facility:(i) The name of the individual;(ii) The UST systems and operator classes to which the individual has been designated;(iii) The date the individual assumed the duties of each operator class; and(iv) The date the individual completed initial training and any required retraining for each operator class.(2) Training of designated operators. Records documenting the initial training and any required retraining of Class A, Class B, and Class C operators must include a certificate of completion. Certificates must include the following information:(a) The name of the trainee;(b) The date the trainee completed the training;(c) The operator class or classes covered by the training;(d) The name of the company providing the training; and(e) For classroom and field-based training, the printed name and signature of the trainer or examiner.[Statutory Authority: Chapter 90.76 RCW.
