Section 16-250-160. Commercial feed license application requirements.

- (1) The commercial feed license application form, to be completed by applicants and licensees, must include:(a) The company name and mailing address of the applicant;(b) The physical address of the facility;(c) The name, contact information, and signature of the applicant;(d) Information regarding the types of business the firm is engaged in (feed manufacturer, dealer, broker); and(e) The type of commercial feed distributed (medicated feed, complete feed, feed supplement, or animal by-products).(2) A commercial feed license is not required for facilities that only:(a) Sell food processing by-products from fruit, vegetable, or potato processing plants, freezing or dehydrating facilities, or juice or jelly preserving plants;(b) Sell bona fide experimental feed on which accurate records and experimental programs are maintained;(c) Makes retail sales of bagged, or packaged commercial feed bearing labeling or other approved indicators showing that the commercial feed is from a licensed manufacturer, guarantor, or distributor who has assumed full responsibility for the required tonnage inspection fees.Note:The commercial feed license application form is available from the department.
